Who is ProActive for?
ProActive is available to individuals who meet a set of eligibility criteria. The criteria have been developed in partnership with clinicians working within Somerset as well as Public Health at Somerset County Council.
To be referred into the ProActive scheme, an individual must be:
- Over 18 years of age
- Registered with a Somerset GP practice
- Ready and willing to make change
- Ideally currently physically inactive and not meeting the Chief Medical Officer’s recommended 150 mins of moderate physical activity per week.
- Diagnosed with one or more medical condition(s)
- Taking medication as prescribed
- Mobile enough to move safely and independently (if the individual requires more one-to-one support when accessing the scheme or facility, they are able to attend with their own support or a carer. Please discuss this when you are contacted by your chosen leisure provider).
In addition to this, there is a list of inclusion criteria that the referring health or fitness professional will need to consider. These criteria ensure that referrers are clear on whether community based physical activity is safe and of benefit for the nature and severity of individuals’ health conditions.
