ProActive Physical Activity on Referral Scheme Criteria

The new ProActive Physical Activity on Referral Scheme has been designed to provide a level of support to individuals that cannot be gained from attending generic physical activity opportunities.

ProActive, in its initial roll out, will cater for the highest risk referrals, with specific leisure providers across Somerset becoming accredited by the ProActive team at SASP to deliver high quality and safe opportunities for participants. Having reviewed the current scheme, it was clear to SASP, the Clinical Governance Group and Public Health that ProActive needed clearer and more robust criteria for health and fitness professionals to be able to refer. The criteria are also required to ensure that leisure providers across the county are operating in line with the 2014 National Institute for Health and Care Excellence (NICE) Guidelines for Exercise Referral and Behaviour Change, and that the pathways in place for exercise referral are aligned to evidence-based principles and best practice guidelines.

The criteria sets out who can refer into ProActive, who is eligible to be referred, the inclusion and exclusion criteria and details on specialist conditions.

Referrers can be confident that the level of guidance, delivery and monitoring offered to an individual participating in the ProActive scheme will be that of at least a Level 3 Exercise on Referral qualification, with some providers employing staff trained to Level 4, providing a higher level of knowledge for particular health conditions.
