ProActive Referral Pathway
SASP has developed a pathway which will act as a guide for referring professionals as well as ProActive leisure providers to ensure that the individual and their referral form is transferred to the chosen centre as smoothly and safely as possible.
It also sets out what to expect within each stage of the process, which can be helpful to discuss with an individual during the initial appointment. Finally, it highlights the stage at which there is an expectation for leisure providers to communicate individual progress and outcomes back to referrers, something SASP understands is an important part of building trust and confidence in the scheme.
